By signing this waiver, I acknowledge and agree to the following terms and conditions in order to participate in any activities, classes, programs, private training, open gym time, events, or use of facilities provided by Boxing With Love(“the Gym”).

Assumption of Risk

I understand that boxing, fitness training, and related activities involve inherent risks, including but not limited to: physical exertion, contact with equipment or other participants, slips or falls, strains, sprains, fractures, head injuries, and other serious injuries.

I voluntarily choose to participate and assume all risks, known and unknown, associated with participation in Boxing With Love activities.

Medical Acknowledgment

I affirm that I am physically able to participate in boxing and fitness activities and that I have no medical condition that would prevent safe participation, or that I have consulted a medical professional prior to participation.

I understand it is my responsibility to stop participation if I experience pain, dizziness, shortness of breath, or discomfort and to notify a coach immediately.

Release of Liability

In consideration of being allowed to participate, I hereby release, waive, discharge, and hold harmless Boxing With Love, its owner(s), coaches, employees, contractors, volunteers, landlords, and affiliates from any and all claims, demands, causes of action, damages, or liabilities arising out of or related to participation, including claims caused by negligence, to the fullest extent permitted by law.

Indemnification

I agree to indemnify and hold harmless Boxing With Love from any claims brought by or on behalf of myself or any minor for whom I am signing.

Youth Participants

If the participant is under 18 years of age, I affirm that I am the parent or legal guardian and consent to the minor’s participation. I agree to all terms of this waiver on behalf of the minor.

Photo & Video Release

I understand that photos and videos may be taken during classes, training sessions, events, or activities.

I grant Boxing With Love permission to use my (or my child’s) likeness in photographs and videos for social media, marketing, advertising, promotional materials, website content, and other lawful uses, without compensation.

If I do not wish for images to be used, it is my responsibility to notify Boxing With Love in writing.

Acknowledgment & Agreement

I have read this waiver in its entirety, understand its contents, and voluntarily agree to its terms. I understand that by signing this waiver, I am giving up certain legal rights.

By signing this agreement, you acknowledge that:

• Membership cancellations require 30 days’ notice submitted through the official website form
• No refunds are provided for memberships or training packages
• Membership payments must be made on time to avoid late fees or termination
• Auto-renewal discounts require a card on file
• Membership freezes are limited, fee-based, and do not replace cancellation requirements

30-Day Cancellation Policy
Cancellation of a membership requires each member leaving Boxing with Love LLC to complete the official Cancel / Freeze Membership Form on the Boxing with Love website at least 30 days prior to the next scheduled payment.

• Failure to complete payments owed or future payments under the enrollment option selected may result in membership collection fees.
• Boxing with Love LLC does not cancel any membership or personal training package via email, text message, phone call, social media, or any form of communication not outlined in this agreement.

No Refund Policy

• Once a gym membership or personal training package is purchased, it is non-refundable.
• No refunds will be issued for any reason, including but not limited to missed classes, schedule changes, travel, injury, illness, or personal circumstances.
• Partial months are not prorated.

Payment Responsibility & Late Fees

• Membership payments are due on the scheduled due date.
• Payments that are 2 days overdue will result in a $25 late fee.
• Memberships that remain unpaid for 2 weeks will result in termination of membership.
• No exceptions will be made.

It is the member’s responsibility to maintain valid payment information at all times.

Auto-Renewal Discount & Card on File Requirement

• Members enrolled in an auto-renewal membership must have a valid card saved on file in Gymdesk to receive the $40 auto-renewal discount.
• Failure to maintain a valid card on file will result in removal of the discounted rate.
• Updating payment information is the responsibility of the member.

Membership Freeze Policy
Membership freezes are available for members who plan to return to Boxing with Love LLC.

• Members may freeze their membership up to two (2) times per calendar year.
• Each freeze month is subject to a $15 monthly freeze fee.
• Freeze requests must be submitted using the official Cancel / Freeze Membership Form on the Boxing with Love website.

Important Freeze Terms:

• Freeze months do not replace or override the 30-day cancellation policy.
• Memberships cannot be canceled during a freeze month.
• If a member chooses to cancel while on a freeze, they will be required to pay one final month at the full membership rate before cancellation takes effect.

Required Method of Communication
All membership freezes and cancellations must be submitted through the official website form. No other form of communication will be accepted, including:

• In-person conversations
• Emails
• Text messages
• Phone calls
• Social media messages
• Messages to coaches or staff

Acknowledgment
By signing below, I confirm that:

• I have read and understand all Boxing with Love LLC membership policies
• I agree to the cancellation, payment, freeze, and refund terms outlined above
• I understand that failure to comply may result in late fees, loss of discounts, or termination
